I've been using a freeware program for many years called Spybot (they do have paid version with added features) and run it periodically. In addition to checking for malware, tracking cookies, etc I've noticed that it updates my HOST file with similar entries as described above. Before running it, I always have it check for updates.
The program can also be configured to run scans automatically.
